We know filling out an insurance form can seem time consuming and difficult. We’ve tried to make it as simple as possible. You know your address and group information, but here is a list of other items you’ll need in order to finish your applications. Print a copy of this list

- List of instruments, equipment, computer hardware and software
- Model/serial numbers, if available, and replacement value for each item
- You can also use this downloadable Excel form to create your equipment list for upload.
- Appraisals are necessary on all vintage equipment regardless of value.

- Studio square footage
- Annual receipts derived from the use of the studio
- Federal Tax ID (if applicable)
- Tour itinerary including dates, venue, and venue capacity (if available)
- Tour transportation details
- Tour payroll (if requesting Workers’ Compensation)
- Certificates of Insurance details from anyone who will be covering you as an “Additional Insured”
- “Additional Insured” information from anyone requesting to be included on your coverage, such as vendors (bus, backline) and tour sponsors. Please refer to your contracts.
Composer & Professional Liability

Auto (Only available to Tour and Business insurance customers.)
- Garaging address
- Make, Model and VIN number
- Driver details (for all drivers) date of birth, license number and expiration
- Previous accidents or convictions for all drivers
